Five Star Management Services, LLC Mascot Naming Contest Official Rules

Rules:Rules: By participating in the Contest, entrant fully and unconditionally agrees to and accepts these Official Rules and the decisions of Five Star Management Services, LLC, which are final and binding in all matters related to the Contest. Five Star employees are not eligible.
Timing: The Contest begins on August 18, 2015 at 12:00 a.m. Eastern Time (“ET”) and ends on August 26, 2015 at 11:59 p.m. ET (the “Contest Period”). Administrator’s computer is the official time-keeping device for the Contest.
Prize: 4 One Direction concert tickets and a VIP suite for Thursday, September 3, 2015 07:00 PM at Ralph Wilson Stadium, Orchard Park, NY.
How to Enter: During the Contest Period,
Make sure you complete and submit the registration form, including a name for the Five Star Mascot. By uploading your Submission, you agree that it conforms to the Submission Guidelines as defined below and that Five Star Management Services, LLC, may disqualify you from the Contest if it believes, in its sole discretion, that your Submission fails to conform to the Guidelines.
- The Submission must not exceed fifty (50) characters in length; and
- The Submission must be in English.
- The Submission must be the entrant’s own original work;
- The Submission must not contain text that is inappropriate, indecent, obscene, hateful, tortious, defamatory, slanderous or libelous; and
- The Submission must not contain text that promotes bigotry, racism, hatred or harm against any group or individual or promotes discrimination based on race, gender, religion, nationality, disability, sexual orientation or age;
- Limit: One submission allowed per person.
Winner Determination: After the conclusion of the Contest Period, a panel of qualified judges determined by Five Star Management Services, LLC in its sole discretion will select the winner based on the following criteria: Creativity/Originality (35%); Wit/Humor (35%); and Relevance to Medical Theme (30%). In the event of a tie, the entrant whose Submission received the highest Creativity/Originality score, as determined by the qualified judges, will be deemed the Contest winner. In the event of a further tie, the entrant whose Submission received the highest Wit/Humor score, as determined by the qualified judges, will be deemed the Contest winner.
Winner Requirements: Without limiting the generality of these Official Rules, the Contest winner shall irrevocably grant, transfer, convey and assign to Five Star Management Services, LLC the entirety of the rights in and to the Submission including intellectual property rights, trademark rights, copyright and all renewals and extensions of intellectual property rights, and the right to secure intellectual property registrations thereto in perpetuity including, without limitation, the rights to use the Submission for any and all purposes in any and all media whether now known or hereafter developed, on a worldwide basis, in perpetuity. The Contest winner accepts and acknowledges that Five Star Management Services, LLC shall not be obligated to use the Submission and that Five Star Management Services, LLC in its sole discretion shall have the right to refrain from using the Submission.
General Conditions: Sponsor reserves the right to cancel, suspend and/or modify the Contest, or any part of it, at any time.
Limitations of Liability: Five Star Management Services, LLC is not for responsible for: (1) any incorrect or inaccurate information, whether caused by entrant or by any of the equipment or programming associated with or utilized in the Contest; (2) technical failures of any kind.
