Five Star Occ Med Physicals
All physicals are conducted by qualified, Five Star Urgent Care medical professionals. While we are proud to accommodate walk-in patients, appointments are recommended for several of the specialized and more in-depth physicals listed.
Employees who are exposed to asbestos may need to have a medical surveillance exam in accordance with OSHA 1910.1001. These include those involved with the mining of asbestos, minerals contaminated with asbestos, maintenance or repair work in asbestos-containing buildings, or direct contact with asbestos-containing waste or dust emissions.
Five Star Occ Med asbestos clearing program includes a full history and examination, review of all questionnaires and, when indicated, X-rays, pulmonary function testing, and blood testing. The program helps ensure that employees are able to perform their job duties safely and can provide early detection of any asbestos-related illness. All asbestos exam protocols are conducted per OSHA standards, but we can add additional testing upon request. Results are reviewed by our on-site Medical Review Officer to provide quick, convenient care.
Our Certified Medical Examiners, included on the FMCSA’s National Registry of Certified Medical Examiners, perform DOT and 19A physicals at all Five Star locations.
Department of Transportation (DOT) Examinations
A DOT examination is a federally regulated medical examination to determine if a person meets the federal standard to drive a commercial motor vehicle. This medical examination includes: vision tests such as distance, color and horizontal field of vision, hearing, dipstick urinalysis, history and physical examination. A DOT physical helps ensure that drivers have no pre-existing medical conditions that will prevent him or her from driving safely. Our NRCME-certified staff members help to ensure all regulations are met, and that drivers are medically qualified to safely and responsibly drive.
Some medical conditions that disqualify or delay receiving a medical certificate:

- Cardiac conditions
- Certain medications
- Neurological conditions

A 19A exam is to a state regulated DOT physical designed specifically for bus drivers. Like the DOT physicals, this exam covers multiple vision tests including distance, color and horizontal field of vision, hearing, dipstick urinalysis, history and physical examination.
Employment physicals help determine if prospective or current employees can perform their work-related duties safely and effectively. These exams include pre-employment physicals, annual and periodic, executive and exit exams.
During these head-to- toe physical exams, our experienced providers will review any current symptoms, past medical history and medications. Depending on the requirements of the position and the employer, our trained medical staff will create an exam that caters to the unique duties of the job. Comprehensive exams may include: whisper tests, visual acuity, blood pressure check and a dipstick urinalysis, but may also include a wider array of diagnostic screenings such as EKG, audiogram, X-ray, pulmonary function test, blood work and a physical fit test.
Five Star performs all firefighter exams according to OSHA standards. We also work with municipalities and other organizations to meet NFPA 1582 standards, if required. Following each exam, you will receive a qualified statement to indicate if the firefighter is medically qualified to perform his or her duties, from the level of fire police to HazMat.
Fitness for Duty Physicals
A Fitness for Duty examination helps create and maintain a safe, efficient workplace. The exam is performed when an employer has specific concerns about an employee’s physical or mental ability to perform the essential duties of a job with or without accommodations. This type of examination is conducted when there is an increase in accidents or injuries, when employee productivity or job performance decreases, or when health problems are observed on the job.
With the collaboration of the employee’s physician, a physical examination will be conducted while reviewing their medical history and job description to create a tailored exam that fits the requirements of the job.
Five Star offers general physicals to help employers determine if prospective or current employees can perform their duties safely. Our providers review any current symptoms, past medical history and current medications. A head-to- toe physical is performed to determine if the employee is able to complete his or her job duties safely and effectively.
These comprehensive exams may also include: height, weight, whisper tests, visual acuity, blood pressure check and a dipstick urinalysis. Depending on the requirements of the position, a wide array of diagnostic screening such as EKG, audiogram, x-ray, pulmonary function test, blood work and a physical fitness test may also be performed to provide a complete assessment of health and wellness.
Return to Work
A Return to Work evaluation is performed to ensure that an employee is able to safely return to work and to perform his or her job duties following an injury or illness. We will collaborate with employers to develop plans for modified duty or additional support, as needed.
The Return to Work exam evaluates the employee within the context of the specific job duties to give the added assurance that this individual is able to perform the necessary tasks of the position. An employee should be released to return to work by his or her attending physician before conducting a return to work exam. It is ideal for the employee to bring a release with him or her to the Return to Work evaluation along with any medical reports from the attending physician.
With busy schedules, it can be a challenge for the parents of student-athletes to find the time to complete required sports physicals prior to athletic seasons. Five Star provides sports physicals at all of our locations on a walk-in basis, with no appointment necessary and online check-ins available. Please bring all of your school’s required forms with you.
This comprehensive physical exam includes reviewing the patient’s medical history in order to evaluate any possible medical concerns. To fully understand a patient’s medical history, questions will be asked about medical conditions, previous injuries, medications, allergies and family illnesses that may impact the patient’s ability to safely participate in sports and athletic activity.

In New York State, students are required to have a physical examination before they enter kindergarten and grades 2, 4, 7, and 10, as well as before entering college. Additionally, students who need working
papers are also required to have a physical exam. At Five Star Occ Med, we can perform those exams and complete the proper paperwork needed to ensure your student can attend school and work.
